First Visit
Your initial appointment will consist of an examination and review of your treatment options.
Please assist us by providing the following information at the time of your consultation:
Any x-rays taken by a previous dentist, if applicable.
A list of medications you are presently taking.
If you have dental insurance, bring your insurance card with you.
New patient forms. If you prefer to complete the forms prior to your visit, the forms can be downloaded here, completed and printed. If you prefer to complete the forms in our office please arrive at least 15 minutes prior to your scheduled appointment time.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian or have consent forms completed and signed by a parent or guardian.
Please alert the office if you have a medical condition (i.e., diabetes, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, anticoagulant therapy, osteoporosis medications, immunosuppressants, etc.) that may be of concern prior to treatment.
X-RAYS
If your previous dentist, a specialist, or physician has taken dental x-rays, you may request that they forward them to our office or you may bring them with you. If Dr. Blomquist determines that additional x-rays are necessary, they will be taken at our office during your appointment.
